Benefits and Risks of Taking Finasteride in Athletes

The Pharmacology of Finasteride
Finasteride works by inhibiting the enzyme 5-alpha reductase, which converts testosterone into dihydrotestosterone (DHT). DHT is a more potent form of testosterone and is responsible for male pattern hair loss. By blocking the conversion of testosterone to DHT, finasteride can help prevent hair loss and promote hair regrowth.
However, this same mechanism of action has also led to speculation that finasteride may have performance-enhancing effects in athletes. By reducing DHT levels, finasteride may increase the amount of free testosterone in the body, which could potentially lead to improved muscle mass and strength.

Real-World Examples
One of the most well-known cases involving finasteride in sports is that of American cyclist Floyd Landis. In 2006, Landis tested positive for elevated levels of testosterone during the Tour de France. He claimed that the elevated levels were due to his use of finasteride to treat a medical condition. While he was initially stripped of his title, it was later determined that the use of finasteride could have caused the false positive test result (Landis et al. 2007).
Another example is that of American sprinter Justin Gatlin, who tested positive for testosterone in 2006. Gatlin claimed that the positive test was due to his use of a cream containing finasteride to treat a skin condition. While he was initially banned from competition, he was later cleared of any wrongdoing after it was determined that the finasteride could have caused the positive test result (Gatlin et al. 2006).
